Email, call my Google Voice service, or fax a note to me.  I prefer email.  Please include in your message a brief reason for your consultation, the name of your current primary care provider, and the best way to contact you (email preferred).

If you use email, please make your message confidential or private using the settings in your email program.  This will assure more privacy in the communications.  If you call me, your voice message will be autoimatically transcribed and sent to me as an email.  Speak clearly and slowly.
